New Client Instructions

If you are a new client and interested in getting started on your healing journey please click here to schedule your free 20min phone consultation to find out if we would be a best fit to work together. I truly value clients finding a match that they feel comfortable with and if that's not me I will let you know! 

Click here for the NEW client portal to complete intake paperwork, enter insurance information and preferred method of payment for co-payments, coinsurance and/or deductible as needed.

 If you are a new client you must complete all steps prior to scheduling your appointment in order for the appointment to be approved (these are also emailed to you after the consultation call) 

1) Review & Sign informed consents

2)  Fill out background information paperwork
3) Enter Insurance information on the portal Please also email me a picture of the back and front of the insurance card to have on file
4) Upload a Payment method on the client portal  for any co-pay/co-insurance/deductible/Late cancel/No Show fees if needed

*****After***** the  above steps are  completed you can then jump to the calendar section of  the portal where you can then schedule your intake appointment at a time that is  convenient to you  (system allows you to make an appointment with a min of 24hours out and  max of up to 2 months out)
